Use this secure form to upload medical records when requesting an initial case review or after completing the Expert Service Agreement for our medical expert witness service.

Note: Please do not send medical records on paper, CD or thumb drive to the AMFS office, as this delays the expert review process. Records received by our office will not be returned.
